If you've been looking for a treatment to improve acne scarring, uneven skin texture, fine lines, enlarged-looking pores or signs of sun damage, you may already have come across fractional CO₂ laser. Fractional CO₂ is an advanced form of ablative laser skin resurfacing.




Rather than simply treating the surface of the skin, it creates controlled microscopic treatment zones, triggering a healing and remodelling response within the skin. Research supports fractional CO₂ resurfacing particularly for concerns such as acne scarring and photoageing.

What Is Fractional CO2 Laser?


A CO₂ laser uses a wavelength of approximately 10,600 nm, which is strongly absorbed by water within the skin. This allows the laser to precisely remove microscopic areas of tissue while generating controlled thermal effects in the surrounding tissue.


The important word here is fractional.

Instead of removing the entire surface of the treated area, fractional technology divides the laser energy into many microscopic treatment zones, leaving areas of untreated skin between them.


This fractional approach was developed to provide the resurfacing and remodelling benefits associated with CO₂ laser while improving the recovery and safety profile compared with traditional fully ablative CO₂ resurfacing.


How Does Fractional CO2 Laser Work?


Think of it as creating thousands of extremely small, controlled columns within the skin.

The treatment produces two important effects.


First, microscopic areas of damaged or aged tissue are removed.

Second, the controlled thermal injury initiates the skin's natural repair process and subsequent collagen remodelling.


This is why results aren't necessarily limited to what you see immediately after healing.


Skin remodelling continues after treatment as the healing response develops. Research into fractional resurfacing suggests this remodelling process is one of the reasons it can improve the appearance of acne scars and skin texture.


What Can Fractional CO₂ Laser Help With?


Depending on the individual, the device and treatment settings, fractional CO₂ resurfacing may be considered for concerns including:


  • Acne scarring

  • Uneven or rough skin texture

  • Fine lines and wrinkles

  • Photoageing and sun-damaged appearance

  • Enlarged-looking pores

  • Certain types of scars

  • Overall skin resurfacing and rejuvenation


Acne scarring is one of the best-established aesthetic applications of fractional CO₂ laser, although results vary according to scar type, severity, treatment parameters and the individual patient.


Is Fractional CO₂ the Same as a Normal Facial Laser?

No — and this is something we want our clients to understand before booking.


Fractional CO2 is an ablative resurfacing treatment. It deliberately creates controlled injury in the skin to initiate renewal and remodelling.


That means there is genuine healing involved. It's therefore very different from having a relaxing facial or a treatment where you can immediately apply makeup and carry on with your day. The stronger the treatment isn't automatically the better treatment either. The appropriate intensity depends on your skin, concern, treatment area, desired result and acceptable downtime.


What Happens During Treatment?

Your journey should begin with a thorough consultation.


Your practitioner will assess your skin, discuss your concerns and expectations, review relevant medical history and determine whether fractional CO₂ is appropriate for you.


On treatment day, preparation and comfort measures will depend upon the device and treatment protocol being used. The laser is then systematically passed across the treatment area to deliver the fractional energy. It is normal for the skin to look and feel significantly different immediately afterwards because the healing process has begun.


How Much Downtime Does Fractional CO₂ Have?

This is probably going to be one of our most frequently asked questions!

Yes — fractional CO₂ has downtime.


Exactly how much depends on the machine, treatment intensity, area being treated and individual healing response. Redness, warmth, swelling, dryness, crusting or peeling can occur following ablative fractional resurfacing. Redness can persist beyond the initial visible healing period in some people.


This is why we'd rather be realistic about recovery than advertise a universal number of “downtime days.” Research also shows recovery and adverse-effect profiles vary between different fractional laser technologies and treatment protocols.

Your practitioner will explain what to expect for your particular treatment protocol during consultation.


When Will I See Results?


Fractional CO₂ isn't necessarily an instant-results treatment. You'll initially go through the healing stage. Once this settles, improvements in the appearance and feel of the skin may gradually become more noticeable.


Because part of the treatment's purpose is to stimulate a longer-term healing and collagen-remodelling response, improvements can continue developing after the initial recovery period. Results and timelines vary considerably between individuals.


How Many Treatments Will I Need?


There isn't one answer for everybody. Someone looking for general resurfacing may have very different requirements from somebody with deeper acne scarring.


Treatment number can depend on: your skin concern, its severity, your skin type, treatment intensity, previous treatments, healing response and desired outcome.


Some clients may benefit from a single treatment, while others may be advised to have a carefully planned course.


We will never recommend more treatment simply for the sake of doing more — your treatment plan should be based on your individual skin.


Does Fractional CO₂ Hurt?


You should expect the treatment to feel more intense than a standard facial.

Comfort measures are normally incorporated into the treatment protocol, but individual pain tolerance varies considerably.


You may also experience heat or sensitivity following treatment.

This is another reason why consultation is important: we want you to understand the treatment itself and the recovery afterwards before deciding whether it's right for you.


Is Fractional CO₂ Suitable for Every Skin Type?

No. This is particularly important with ablative laser treatments.


Skin type, tanning, medical history, medications, previous pigmentation problems, active skin conditions and the concern being treated can all affect suitability.

One recognised complication is post-inflammatory hyperpigmentation (PIH), where the skin develops increased pigmentation following inflammation or injury. The risk can be particularly relevant in darker or pigment-prone skin types.


That doesn't mean everyone with a darker skin tone automatically cannot have fractional laser. It means careful assessment, appropriate technology, conservative parameters where indicated and proper pre- and post-treatment care matter enormously.


What Are the Possible Risks?


Fractional CO₂ is an advanced treatment and should be presented as such.


Potential side effects and complications can include: redness, swelling, discomfort, crusting or peeling, prolonged redness, temporary pigment changes, post-inflammatory hyperpigmentation, hypopigmentation, infection and, less commonly, scarring.


Individual risk varies, which is why medical history, skin assessment, treatment parameters and aftercare are so important. Fractional resurfacing generally has a more favourable recovery profile than traditional fully ablative CO₂ resurfacing, but it is not risk-free.


What About Pigmentation and Melasma?


This needs particular care. Fractional CO₂ can be used in selected pigment-related contexts, but melasma is complex and should not simply be advertised as something CO₂ laser will remove.


Melasma can be influenced by hormones, UV exposure, visible light, heat and inflammation, and aggressive treatment can sometimes make pigmentation worse.


If pigmentation is your main concern, we need to identify what type of pigmentation we're actually looking at before deciding on treatment. Sometimes not using the strongest treatment available is actually the smartest treatment decision.


Why Is Autumn a Great Time to Talk About Skin Resurfacing?


After summer, many clients begin thinking about repairing and refreshing their skin.

Laser resurfacing also requires particularly careful attention to sun exposure and post-treatment protection, so the cooler, less sunny part of the year is often a more practical time for many people to consider a resurfacing programme.


That does not mean SPF disappears when summer does. Daily broad-spectrum sun protection remains an essential part of protecting your skin and is especially important when undergoing treatments that affect the skin barrier or pigmentation.
